Deep Brain Stimulation

Treatment-resitant depression  is a seriously impairing disorder with no  treatment choices once numerous medications, psychotherapy, and electroconvulsive treatment have fizzled. In view of our fundamental perception that the subgenual cingulate region is metabolically overactive in treatment-resistant depression, we examined whether the use of deep brain stimulation  to modulate BA25 could diminish this raised action and produce clinical advantage in six patients with refractory depression. Chronic stimulation of white matter tracts contiguous the subgenual cingulate gyrus was related with a striking and supported reduction of depression  in four of six patients. Antidepressant effects were associated with a marked reduction in local cerebral blood flow as well as changes in downstream limbic and cortical sites, measured using positron emission tomography.
