Resistant Hypertension Review Articles

Resistant hypertension is defined as above-goal elevated blood pressure (BP) in a patient despite the concurrent use of 3 antihypertensive drug classes, commonly including a long-acting calcium channel blocker, a blocker of the renin-angiotensin system and a diuretic. The antihypertensive drugs should be administered at maximum or maximally tolerated daily doses. Resistant hypertension also includes patients whose blood pressure achieves target values on ≥4 antihypertensive medications. The diagnosis of Resistant hypertension requires assurance of antihypertensive medication adherence and exclusion of the white-coat effect . The importance of Resistant hypertension is underscored by the associated risk of adverse outcomes compared with non-Resistant hypertension. Resistant hypertension is an elevated ambulatory blood pressure after exclusion of secondary hypertension in patients who are fully adherent to treatment with antihypertensive drugs. Resistant hypertension is associated with increased risks of adverse health outcomes, including cardiovascular and renal events and all-cause mortality
