Abstract

This study examines the impact of pharmacist-led medication therapy management (MTM) on patient adherence and health outcomes. MTM involves pharmacists working closely with patients to optimize medication regimens, enhance adherence, and improve health outcomes. The objective of this research is to evaluate the effectiveness of pharmacist-led MTM interventions in enhancing patient adherence and improving health outcomes. The study employs a systematic review approach to analyze relevant literature and identify key findings. Pharmacist-led medication therapy management (MTM) has emerged as a promising strategy to address this issue. MTM involves pharmacists working closely with patients to optimize medication regimens, educate patients on proper medication use, and provide ongoing support to enhance adherence and improve health outcomes [1].

This research aims to evaluate the impact of pharmacist-led MTM interventions on patient adherence and health outcomes. Pharmacists are highly trained healthcare professionals who possess extensive knowledge about medications and their effects on the human body.
